Required Forms & Immunization Documents

All day students at the traditional campus are required to complete TB screening questionnaire and other online forms using the Student Health Portal. Students must submit proof of two vaccinations for measles, mumps and rubella (MMR) and one for Meningitis. You may also upload documentation of COVID-19 vaccinations. See student immunization policy.

Failure to submit these documents to Health Services will prevent students from registering for subsequent semesters.

Log into the Student Health Portal Secure to complete all necessary steps.

Our staff facilitate interactive presentations on a wide variety of health topics throughout the year to student groups, athletic teams, classrooms, residence halls, faculty and staff. Learn how to make a positive difference and help prevent suicide on campus and across out state and nation.

Here’s how, using the Columbia College Ask Listen Refer Training Program:

CC faculty, staff, and students can help prevent suicide by learning to:

  • Identify people at risk for suicide
  • Recognize the risk factors, protective factors, and warning signs of suicide
  • Respond to and get help for people at risk

The Ask Listen Refer online training is a free program that takes about 20 minutes to complete. It is intended to be educational rather than therapeutic, and it must be completed in one sitting. Logging in and out of the program may cancel your progress and prevent you from obtaining your certificate of completion.

BASICS stands for Brief Alcohol Screening and Intervention for College Students. BASICS is a non-judgmental program that evaluates students' personal alcohol and drug use. The program increases student motivation to be safe and reduce harm with alcohol use.

Information shared by a student in BASICS is confidential. If you are referred by another office, you maybe asked to give written permission for Wellness Center staff to inform the referrer of your attendance. If you are curious about your drinking habits, call and make an appointment today. 

Nicotine cessation is a series of non-judgmental consultations that help students gain personal motivation to cease nicotine use.  For students who want to decrease or quit their nicotine use, they may meet with a professional to obtain resources, learn techniques for coping with cravings, make a plan for quitting, learn strategies for staying nicotine free and cope with relapse. Call to  schedule a consultation.

Wellness weeks provide time to engage students with fun activities, meet our staff, gain knowledge and learn about resources.  They vary from year to year depending on student needs. Examples of past programming include Sexual Abuse and Domestic Violence Awareness, Suicide Prevention and Safe Spring Break.
