Report an academic integrity violation

Academic violations are behaviors that include, but are not limited to, cheating, plagiarism or falsifying official records related to the College. General classroom misconduct is considered a non-academic violation. To report academic misconduct, please use the Academic Misconduct Reporting form .

Report a non-academic violation/behavioral misconduct

Non-academic violations are behaviors that include, but are not limited to, disorderly conduct (verbal or physical), alcohol and/ or other drugs, vandalism or other violations of College policies, regulations, or rules. To report non-academic misconduct please use the Incident Reporting Form (non-academic) .

Student of Concern Referral: Report concerning or threatening behaviors: 

Faculty, staff, students, and guests are encouraged to use the Care and Concern Behavioral Referral Form to report behavior that they perceive as concerning, worrisome, or threatening. All referrals will be reviewed by the Student of Concern Action Team comprises representatives from Campus Safety, Student Affairs, Residential Life, Student Conduct, and Wellness Health and Counseling. 

Report a Title IX incident

The Title IX Coordinator, Erin Mazzola, or a Deputy Title IX Coordinator, reviews all reports of as they pertain to the College’s Title IX and Sexual Harassment Policy. If you have any questions pertaining to a Title IX incident please reach out to Erin Mazzola, TitleIXCoordinator@CCIS.edu or call (573)875-7690.  To report interpersonal violence including, dating violence, domestic violence, sexual harassment, sexual misconduct or stalking behaviors please use the College’s Title IX Sexual Harassment Incident Report .

Do you have information and/or a concern for the Office of Student Success?

General academic, attendance, financial, retention and student success concerns should be submitted through the through the Early Alert system in Aviso. This could include a student missing classes, not turning in homework, disengaged with class work, expressing financial struggles, sharing they may not return to CC, or discussing how they are feeling overwhelmed with school.

Pregnancy and Parenting Accommodation Request Form for students

Columbia College is committed to creating an accessible and inclusive environment for pregnant and parenting students. Title IX of the Education Amendment of 1972 prohibits discrimination on the basis of sex that occurs within its education programs and activities. Among the types of sex discrimination covered by this statue, Title IX protects against discrimination related to pregnant AND parenting students.

The purpose of this form is to assist the College in determining whether, or to what extent, a pregnancy adjustment can be granted. Pregnancy and Parenting Accommodation Request form for Students .
