Immunization Requirements at Columbia College

 

Required forms & immunization documents

All day students at the traditional campus are required to complete TB screening questionnaire and other online forms using the Student Health Portal on myPortal. Students must submit proof of two vaccinations for measles, mumps and rubella (MMR) and one for Meningitis. You may also upload documentation of COVID-19 vaccinations. See student immunization policy.

Failure to submit these documents to Health Services will prevent students from registering for subsequent semesters. For more information, contact  The Wellness Center.
