Mid-term/Final Proctor Reservation Form - NS Everett - Marysville (Marysville Campus Office)

Note: All fields (except Comments) are required.

Mid-Term and Final Exams for Columbia College Online courses are proctored during Week 4 and Week 8 of each Term by APPOINTMENT ONLY.  Please review the proctoring schedule before submitting a reservation request:

* Marysville Campus Office: W/Th (9:30, 11:30, 1:30 or 3:30 PM); Friday (9:30, 11:30, and 1:30 pm).  Saturday (9 AM, 11 AM, 1 PM).  Saturday proctoring reservations must be made before Close of Business Monday of Week Four and Week Eight.

* Naval Station Everett Office:  W/Th/F (9:30 AM or 11:30).  Students must have access to NavSta Everett as campus cannot escort students onboard base.
