Mid-term/Final Proctor Reservation Form - Ft. Drum

Note: All fields (except Comments) are required.

If you are planning to take two exams, you must fill out two registration forms and select different times and/or days to take the tests. Testing will be completed in the Education Center on Fort Drum.

Midterm exam dates open during the 1st week of classes.                            Final exam testing dates open Monday of the 5th week of classes.
