Mid-term/Final Proctor Reservation Form - Freeport

Note: All fields (except Comments) are required.

If you are planning to take two exams, you must fill out two registration forms or take the tests different days. Testing will be completed in the following locations on Highland Community College campus:

Thurs.:   Building H (Student/Conference Center) 127, 5:30 p.m.

Sat.:        Building R (Community Services Center) 103, 9:00 a.m.

If you wish to sign up for a midterm and a final, you must fill out two separate requests.
