Mid-term/Final Proctor Reservation Form - NWS Charleston

Note: All fields (except Comments) are required.

Welcome Columbia College, Joint Base Charleston Weapons Station, students! Please register for your exam below. Major Field Tests (MFTs) are administered only after the fifth week of classes by appointment only. Ensure a separate form is used for each exam. Please remember to bring your I.D and be prompt for all appointments. Schedule exam reservations using the proctor reservation form two weeks prior to testing.
