Mid-term/Final Proctor Reservation Form - Hunter Army Airfield

Note: All fields (except Comments) are required.

Please sign up for your exam(s) below, and complete ALL fields.

Your proctor will contact you via email to confirm your test date and time. Please provide the correct cougarmail address to ensure receipt of  confirmation for your proctor reservation. Proctor exams begin promptly at 10:00 AM and 2:00 PM. Late examinees will be asked to reschedule for a later date and time.

Children are not allowed during proctored exams. No exceptions.
