Mid-term/Final Proctor Reservation Form - Fort Leonard Wood

Note: All fields (except Comments) are required.

 IMPORTANT: You are only allowed to take one exam per time slot. You may only reserve one test slot at a time using this form. If you wish to sign up for a mid-term and a final, you must fill out two separate requests.

  • PLEASE BE SURE to type in your email address correctly: without it, we cannot email you with a confirmation of your reservation.
  • BE SURE of your Student ID number.
  • If you are unsure of your course prefix, number, and section this information can be found via CougarTrack under Class Schedule
  • PLEASE verify the spelling of your instructor's name for this course!
  • Allow 8 business hours for processing of your reservation and a confirmation to be sent to you.
  • Arrive 15 minutes prior to your scheduled exam time and doors will close at start of posted time.
  • Please remember to bring a valid ID card with you when you arrive for your exam.

  • MFT exams must be scheduled by contacting Brian Bilderback at (573) 329-4050 or Gary Robinson at (573) 336-7028. Do not use this form for MFT exams.
