Employee Performance Evaluation Forms

There are now two different Columbia College performance evaluation forms — one for supervisory staff and another for non-supervisory staff.

• Performance Evaluation – Non-Supervisory Staff
• Performance Evaluation – Supervisory Staff

They use the "forms" feature of Microsoft Word.  You can move between the blanks on the form by using the "TAB" key.  To place a mark in a check-box, either TAB to that box and press the SPACE BAR, or click on that box with your mouse.
