Change Lender Request Form

• This notice is used to ensure that the Financial Aid Office provides sufficient service to you through the process of changing lenders.
• This is a notice for Financial Aid only; students must also complete a new promissory note.
• By completing this form you are requesting that any funds that have yet to disburse under your current lender be cancelled and funds be processed under the new lender.
• Any processing comments will be directed to your Cougar Mail account.
